In this section you will be asked to complete the following forms:

1. Medical Authorization
2. Communicable Disease Waiver
3. Random Drug & Tobacco Testing (RDT)
4. Concussion and Sudden Cardiac Arrest Acknowledgement and Signature Form for Parents and Athletes
5. Photo and Video Release
6. Release of Liability
