travel AGREEMENT

    The Center Grove Ultimate Club has established the following rules for players participating in Club sanctioned activities requiring travel and overnight hotel stays:

    • Players will only travel to out of state tournaments with a parent, coach or chaperone.  Players may not drive and transport other players to out of state tournaments or transport other players during the trip.  
    • Players will only leave the Club's designated hotel with a parent, a member of the Club's designated coaching staff or a certified chaperone; no exceptions.
    • Players will only be in an opposite gendered room, during free time determined by the Club's designated coaching staff, with doors open at all times.
    • Players will except personal and financial responsibility for actions resulting in damage to property during travel to, during and travel from any Club sanctioned activities. 

    I understand and agree my electronic signature below acknowledges that I will adhere to the above travel rules at any Club sanctioned activities requiring travel and overnight hotel stays.  I further understand that my failure to follow these rules may result in corrective actions including, but not limited to, suspension or expulsion from the Club, school notification and/or notification to applicable public safety officials.
