If you have not played with CG Ultimate Club this academic year (fall, winter, spring seasons), then please click on the Google forms document to the left to complete your registration documents.

Players who completed registration forms already during the 2023-2024 school year do not need to complete these forms.

Forms take approximately 10-15 minutes to complete.
Registration must be completed by a parent / legal guardian if player is <18 years of age.
