Registration forms take approximately 10-15 minutes to complete.
Registration must be completed by a parent / legal guardian if player is <18 years of age.

Registration forms include:

• Personal Information
• Medical Information & Medical Waiver
• Participation/Liability Waiver
• Photo/Video Release
• Health Physical Form

Once you have completed your registration forms, click below to pay for registration:

NEW PLAYERS: click here to pay for new player registration ($240)
(New players receive USA Ultimate membership & a reversible tank)

RETURNING PLAYERS: click here to pay for returning player registration ($165)



When your registration forms are submitted ​and payment complete -- you are done!

Remember to submit your health physical form ASAP.  This must be submitted before you can join the team for practices.
