All players are required to have a Physical Examination form, completed April 1, 2022 or after, and on file with the Center Grove Ultimate Club (CGUC) prior to participation in any CGUC sponsored activity for the Club year (August 1 - July 31).

- Schedule an sports physical appointment with a health care professional holding an unlimited license to practice medicine, a nurse practitioner or physician assistant.
- Bring the form to the appointment and provide to the health care professional to complete.
- Following completion of the sports physical, scan the completed form and email the form to Mandi Harmon, RN & CGUC Health & Wellness Director at mharmon@centergroveultimateclub.org
