Before beginning registration, please note the following:

USA Ultimate (USAU) Account: 

• Center Grove Ultimate Club (CGUC) is an affiliate of USA Ultimate.  You will be required to provide your USAU Account ID as a part of registration.
• If you already have a USAU Account ID, click on the USA Ultimate logo to login and obtain your USAU Account ID.  *Do not create a new USAU Account.*
• If you do not have a USAU Account ID, then click on the USA Ultimate logo to join.  You can create a free account.  Do not pay for membership, as this will be covered by CGUC.  Click here for the "Create a USAU Account Step-by Guide".
• Verify that you have completed the annual USAU Release of Liability Waiver

registration forms

***If you completed registration forms in the Fall, you do not need to complete them for Winter.***

Registration forms take approximately 10-15 minutes to complete.
Registration must be completed by a parent / legal guardian if player is <18 years of age.
