I am aware that participation in Center Grove Ultimate Club, Inc. (CGUC) approved events including, but not limited to, to programs, games, and tournaments, may result in personal injury and/or other loss or destruction of property. I hereby waive and release any and all claims against the CGUC and Center Grove School Corporation, it's officers, members, staff, employees, agents, and volunteers arising out of, or relating to, participation in CGUC approved events and/or the provision of emergency medical treatment, now or in the future, to the extent permitted by law, including without limitation claims for personal injury or death, loss of or damage to property, and related costs and expenses.
